Billing, Payment, and Insurance Guidelines

This policy outlines the procedures for billing, payment collection, insurance claims processing, and client financial responsibility. The objective is to ensure consistent financial practices, transparency with clients, and compliance with applicable healthcare regulations.

Insurance Billing


  • Accepted Insurance
  • Crosspoint Counseling Center accepts select insurance plans. When applicable, claims will be submitted to the client’s insurance provider on their behalf.

  • Insurance Verification
  • Prior to the first appointment:
  • Administrative staff will verify insurance eligibility and benefits when possible.
  • Verification is not a guarantee of payment or coverage.


  • Clients are ultimately responsible for all charges associated with services received.

  • Client Financial Responsibility

  • Clients utilizing insurance are responsible for:
  • Co-payments
  • Coinsurance
  • Deductibles
  • Non-covered services
  • Services denied by the insurance carrier


  • All applicable amounts are due at the time of service unless prior arrangements have been approved.

  • Usual and Customary Rates
  • All claims are submitted using the Center’s Usual and Customary (U&C) rates. Insurance reimbursement may vary based on the client’s specific policy.

Private Pay (Self-Pay) Services

  • Clients may elect to receive services without using insurance.

  • Self-Pay Rates
  • Services are billed according to the established private pay rate schedule, including:
  • Individual sessions
  • Couples sessions
  • Family sessions

  • Payment is due at the time of service unless otherwise approved.

  • Clients may request a superbill for potential out-of-network reimbursement through their insurance provider.

Payment Methods

  • Crosspoint Counseling Center accepts:
  • Credit and debit cards
  • Health Savings Accounts (HSA) / Flexible Spending Accounts (FSA)
  • Cash and personal checks (payable to the clinician

  • Payment information may be securely stored within the Center’s electronic systems for convenience and recurring billing.

Payment Collection

  • All co-pays, deductibles, and self-pay fees are due at the time of service.
  • Clients may be required to maintain a valid card on file.
  • Outstanding balances may be charged to the card on file if not resolved within the designated timeframe.

Missed Appointments and Late Cancellations

  • A minimum of 24 hours’ notice is required to cancel or reschedule an appointment.
  • Appointments canceled with less than 24 hours’ notice, or missed without notice, may incur a late cancellation or no-show fee.
  • These fees are the sole responsibility of the client and are not billable to insurance.

Sliding Scale / Scholarship Program

  • A limited Sliding Scale / Scholarship Program is available for clients experiencing financial hardship.
  • Completion of a Scholarship Application is required.
  • Approval is based on financial need and program availability and is not guaranteed.
  • Reduced-fee services are limited to:
  • Six (6) sessions per approval period
  • Twelve (12) sessions lifetime maximum

Outstanding Balances

  • Clients are expected to maintain a current account balance.

  • The Center reserves the right to:
  • Establish payment plans for outstanding balances
  • Suspend future scheduling until balances are resolved
  • Refer delinquent accounts to collections after reasonable collection efforts


Sliding Scale / Scholarship Program

  • Crosspoint Counseling Center reserves the right to modify fees and billing policies. Clients will receive reasonable notice prior to any changes.
